A Note on downloadable forms:
Application forms related to one's voting status may be downloaded, completed
and mailed to our office for processing. However, in view of the fact that a
digital photograph needs to be taken and that the new Identity Card needs to be
signed by the individual, forms related to identity cards must be presented
personally at our office after being downloaded and completed.
